You can use your vision benefits once every 12 months, beginning on the date you first use them.
Using out-of-network providers
If you use an out-of-network provider, you’ll pay for services out of pocket and submit a reimbursement request. Reimbursement amounts are shown in the table below.

Paying for vision care expenses
If you’re enrolled in either PPO medical plan, you can contribute pre-tax to a Healthcare Flexible Spending Account (FSA) to pay eligible vision expenses.
If you’re enrolled in the HDHP medical plan, you can pay eligible vision expenses using your Health Savings Account (HSA).
Cost for coverage
You pay for vision coverage through semi-monthly paycheck deductions. The amount you pay is determined by the plan you choose and who you cover.
How to enroll
You can enroll in vision insurance during Open Enrollment through Employee Navigator , as a new hire, or if you experience a qualifying life event.
Coverage begins the first day of the month following the date you were hired or became eligible for benefits.
