Eligibility

Review Lumexa Imaging eligibility rules, eligible dependents and benefit changes after enrollment.

Employees

If you are a benefits eligible employee of Lumexa Imaging, most benefits are effective on the first day of the month following your date of hire.

Dependents

Eligible dependents are defined as:

  • Your legal spouse (or common-law spouse when recognized)
  • Children up to age 26, regardless of dependency, student or marital status
  • Children 26 or older who are fully dependent on you for support due to a mental or physical disability and are indicated as such on your federal tax return

Changes to eligibility

If you or an eligible dependent gain or lose eligibility for benefits during the year, you may be able to make changes to your Lumexa Imaging benefits.

Working spouse surcharge

Lumexa Imaging reserves the right to verify whether your spouse is provided coverage elsewhere.

Tobacco/vaping surcharge

If you smoke or vape and enroll in the Lumexa Imaging medical plan, you will pay a monthly $100 tobacco surcharge. Ready to quit? Contact your Clinical Advocate at 1-800-876-9835 for guidance on where to start.
