- Silver Plan
- Gold Plan
Understanding your dental plan options
With a focus on prevention, early diagnosis and treatment, dental insurance can greatly reduce your costs when it comes to restorative and emergency procedures.

In-network vs. out-of-network
While you have the choice of seeing any dental provider, you will always save money by staying in-network. Out-of-network reimbursement is based on Usual, Customary, and Reasonable (UCR) fee guidelines, not pre-negotiated charges. You will be responsible for billed amounts above UCR when choosing out-of-network providers.
To find an Ameritas network dentist, select “Classic (PPO)” from the network drop-down.
Compare coverage details
The table below shows in-network coverage. Out-of-network coverage levels are the same, but claims are paid based on UCR fee guidelines, so you may pay more. For full coverage details, view the plan summary for each plan on the Documents & Forms page.
The deductible is the amount you pay before the plan starts to pay. The calendar year maximum is the most the plan will pay for covered expenses each plan year. These limits reset annually on Jan. 1.
The lifetime maximum for orthodontia is the most the plan will pay per person for covered orthodontia services.

Paying for dental care expenses
If you’re enrolled in either PPO medical plan, you can contribute pre-tax to a Healthcare Flexible Spending Account (FSA) to pay eligible dental expenses.
If you’re enrolled in the HDHP medical plan, you can pay eligible dental expenses using your Health Savings Account (HSA).
Cost for coverage
You pay for dental coverage through semi-monthly paycheck deductions. The amount you pay is determined by the plan you choose and who you cover.
How to enroll
You can enroll in dental coverage during Open Enrollment through Employee Navigator , as a new hire, or if you experience a qualifying life event.
Coverage begins the first day of the month following the date you were hired or became eligible for benefits.
